A Clinical Study in a Case of Bilateral Pheochromocytoma


Abstract
A 18 year old boy with suprarenal [pheochromocytomas on both the left and right sides bilaterally was treated preoperatively with oral administered phenoxybenzamine hydrochloride and propranolol hydrochloride.
The patient had had hypertension for six months prior to admission.
Blood pressure on admission was 250 mmHg systolic, 180 mmHg diastolic with grade I hypertensioe f etinopathy. The cardiac rate was 100 to 110 beats per minute. The regitine test yielded a reduction of systolic pressure in 60 mmHg and diastolic pressure in 55 mmHg. Urinary catecholamine in 24 hour urine was 13740 ¥ìg. .
The tumors were removed with abdominal approach and 48 gm tumor was removed from the right suprarenal area and 59 gm tumor from the left suprarenal area.
A significant arrhythmia and premature ventricular beat occurred at the time of manipulation of the tumor which were well and easily controlled with the administration of 2 mg of Inderal. No significant elevation of blood pressure was noted only except mild elevation with tumor manipulation which was well controlled with the administration of 5 mg of regitine.
We suggest that the patient was protected from ?-adrenergic mediated hypertension with the use of phenoxybenzamine but not fully protected from ?-adrenergic mediated cardiac arrhythmias because of two small amount and short duration of ?-adrenergic blockade, propranolol. The follow up result of 7 months after operation has been sactisfactory. In addition, correlated literature was also reviewed.
